THE USA

The Health Care System in the USA is organized in three levels : family doctor, the medical institution or hospital and the US Public Health Service.

A family or private doctor gives his patients regular examinations and inoculations.

In case when professional service and care is needed the family doctor arranges for the specialist or a hospital for his patients.

The family doctor receives pay directly from the patient.

Most physicians have private practice.

But many Americans have no family doctor and they come directly to the hospitals for their medical needs.

There are government - financed and private hospitals.

The patients are admitted to hospitals or clinics staffed by consulting physicians, residents, interns and highly skilled nurses.

Most hospitals have at least the following major medical departments or units : surgery, obstetrics and gynecology, pediatrics and general medicine.

Emergency units are very special in the hospitals.

Emergency patients acquire immediate attention.

The cost of medical care in the USA is very high.

Most of the population (75%) have their health insurance, life insurance, disability protection and retirement benefits at their place of employment.

TEXT 8.

THE LAW AND THE FAMILY

Here, we will look at how the law sees the family a special institution ; how some legal systems treat married couples and their children differently from the unmarried ; the process of divorce ; custody of and responsibility for children ; and protection from violence in the home.

Finally, we will consider the rights of succession to the property of a family member when they die.

Beyond the mere function of providing a new generation of children, the family is often promoted for its moral contribution to society.

Despite a growing labor shortage, the Japanese government has passed very little legislation challenging the assumption that mothers should stay at home rather than go out to work.

In Ireland, which is strongly influenced by the doctrines of the Catholic Church, divorce remains illegal.

In some societies the family is thought to be so important that there is very little legal intervention in family life.

In many Islamic countries, for example, fathers, brothers and sons are allowed considerable authority over the females in their family.

As late as the 1970s, the male head of the household in Switzerland was deemed to represent the interests of everyone within that household, and, consequently, none of the women could vote in national elections.

But in many parts of the world, the law now promotes the rights of individuals within the family unit, and regulates family relations through legislation.

Raised from the taxes of the working population as a whole, child benefit is paid directly to the mother, and retirement pensions are paid to grandparents, so that they are less dependent upon financial support from a family member.

In Sweden, parents can be prosecuted for physically punishing their children and children have a limited capacity to divorce their parents.

In Britain, as in many countries, there are special family courts with very strong powers to control and transfer private property in the interests of children.

Much of the work of other courts is also directly relevant to family life.

In 1969, the Queen wanted to 'open a window' on to the royal family.

That year television cameras came into the royal home and filmed the family in private for the first time.

Forty million people watched the programme.

They saw a different side of the royal family.

The programme brought the royal family closed to people, but it also helped to make problems for the future.

Now the public wanted to see more and more of royal private life.

It was harder to keep the reporters out.

In the last thirty years there have been a lot of documentaries, series and interviews about the royal family.

Before the days of television they seemed formal.

Today they seem informal and friendly.

As well as TV programmes, there are also newspaper stories, books and magazines about the Queen and her family.

Hundreds appear every year.

Some are good - many are bad, but almost all of them are popular.

The earliest social welfare services in Britain were provided by various religious orders, augmented in medieval times by the manor houses and merchant and craft guilds, which assumed as part of their duties and responsibilities the care of the sick and the desti¬tute.

This practice fell into disuse with the decay of the feudal system and the dissolution of the monasteries.

By the end of the sixteenth century it had become imperative to find some substitute for the old system.

In 1601, therefore, the Poor Relief Act gave local government authorities in England and Wales the duty to provide from local taxation for the sick, the needy, and the homeless.

(A similar Act had been passed in Scotland in 1579.

) Local authorities also began to take some steps to control water supplies and to try to check epidemics.

In the eighteenth and nineteenth centuries British medical services developed.

Medical science advanced and the number of qualified doctors greatly increased.

Hospitals were built with private endowments and subscriptions and were made increasingly available on a charitable basis to the general population, while free 'infirmaries' were provided under the Poor Law for the destitute, aged and infirm.

From the middle of the nineteenth century working people of modest means began increasingly to insure against periods of illness by subscribing to provident and friendly societies and sick clubs.

Many public - spirited medical practitioners would at that time remit all or most of their fees to the poor.

The National Health Service provides free treatment for people living in Britain and gives emergency treatment for visitors.

The greater part of the cost is met from taxes taken from people’s wages.

People also pay some money every month as a sort of insurance.

The National Health Service consists of three main parts : the general practitioners, the hospital and specialist services, and local health authority services.

Local health authorities are responsible for medical education, hospital building, environmental health, vaccination service and so on.

The centre of National Health Service is the general practitioner (GP).

Each person is registered with a certain doctor in his or her area.

The GP diagnoses, gives medical certificates, prescribes medicines.

Dentists and opticians usually have separate clinics.

They are not parts of health centres.

There is also a medium - level hospital staff.

District nurses give injections, physiotherapy exercises at people’s homes.

Ward nurses take care of the ill in the hospital.

Regular medical inspections are held at schools.

Children receive various vaccinations and are examined by different specialists.

There also exists a school dental service in every school.

Much attention is paid to the educational programmes.

The Department of Health provides anti - smoking education programmes, alcohol education programmes, cancer prevention programmes and so on.

Much attention is paid to the AIDS and drug programmes.

Great Britain pays much attention to the qualification of doctors.

They are trained at 16 universities.

Besides, they get practice during their work at teaching hospitals.
